Promoting Treatment Engagement with Behavioral Health Disorders

Substance use and mental health disorders are widespread problems in the United States to the extent that nearly 1 in 4 Americans meet the diagnostic criteria for one or the other in a given year (Kazdin, 2017). Most recent national estimates suggest that in 2011, 2.5 million emergency department (ED) visits resulted from medical emergencies involving drugs and alcohol, a 52% increase since 2004 (SAMHSA, 2011a). Despite these high rates, conservative estimates suggest that between 50-70% of those who qualify for substance abuse or mental health treatment do not actually receive it (Satcher & Rachel, 2017). When it comes to substance use and other behavioral health treatments, the dictum, “Build it and they will come” is simply not accurate. In other words, the vast majority of individuals with substance use and other behavioral health conditions will not seek and engage in treatment unsolicited. Nevertheless, such an approach has been the dominant model of behavioral health service delivery in the United States. For the vast majority of treatment programs, services and approaches are configured to address problems in individuals who are assumed to be ready to make changes in their lives. Strategies for successful engagement of clients, especially those who are either not ready or willing to change, are desperately needed. While there are significant systemic barriers to engagement including the proximity and accessibility of services and high costs of care (Kazdin, 2017), treatment providers also influence the extent to which clients engage in treatment. This course is intended for licensed clinicians and will provide information about the challenges of engagement, as well as specific strategies that providers can use to promote engagement in treatment for individuals with substance use and other behavioral health conditions.
